- Address Change Form – Current students must submit address changes via MyMercer
- Authorization to Release Information
- Course or Term Withdrawal Form – All withdrawals must be submitted electronical via MyMercer by selecting the Withdrawal Form under ‘Academics’.
- Enrollment Certification Request Form
- Independent Research & Writing Form
- Name Change Form – Current students must submit name changes via MyMercer
- Non-Returning Form – The non-returning form must be submitted electronically via MyMercer by selecting Non-Returning Form under ‘Academics’.
- Replacement Diploma Order Form
- Request for a Letter of Good Standing
